The Diabetes Initiative National Program Office (NPO), funded by The Robert Wood Johnson Foundation, is housed in the Division of Health Behavior Research at Washington University School of Medicine in St. Louis, Missouri. The NPO provdes oversight and technical assistance for two national diabetes self management programs: Advancing Diabetes Self Management and Building Community Supports for Diabetes Care. These two programs include 14 demonstration projects that test and introduce quality improvements in the resources and supports for diabetes self management in primary care and community settings, respectively. The NPO facilitates learning and quality improvement among the grantees through the use of a collaborative learning network, workgroups, expert consultation and focused training.
